Associates in Medical Physics, LLC, (AMP), agrees to supply the following services:

Perform performance evaluation and compliance testing of each C.T unit with respect to the following:




Accuracy of positioning lasers/lights.

Slide thickness accuracy.

Table-incrementing accuracy.

Noise levels of the system in the Head and Body modes, and at various slice thicknesses.

Image uniformity of the system in the Head and Body modes.

High contrast resolution in the Head and Body modes, and at various slice thicknesses.

Low contrast resolution in the Head and Body modes, and at various slice thicknesses.

Determination of contrast scale (i.e., change in linear attenuation coefficient with C.T. number).

Determination of C.T. number linearity (i.e., establish constancy of contrast scale over the range of C.T. numbers of clinical interest).

Verify relative independence of absolute C.T. number of water with respect to subject size and kVp.

General image quality with regard to imaging artifacts.

General safety of the unit.




Perform dosimetry measurements of each C.T. unit with respect to the following:




Physical location in an acrylic phantom (32cm Body or 16cm Head)

kVp

mA

Scan time

Scan diameter

Slice width




Provide a typed report of all findings for use as a permanent record, in a format acceptable to JCAHO and AOA.
